People keep a lot of fucked-up shit in their heads. If they act relatively normal, or at least don't draw attention to themselves, it's often difficult for anyone to know it's there.
I think it's very fortunate that Chism was so brazen and shortsighted when he decided to turn his fucked-up thoughts into fucked-up reality. I personally don't doubt he'd have gotten better at it and kept killing people if he hadn't been stopped after his first kill.
